Community Care Plan and CareSignal Launch Deviceless Remote Monitoring Service to Increase Access to Care for Vulnerable Members

January 19, 2021 at 11:13 AM

CC-logo-FINAL-high res(FLORIDA, January 19, 2021): Community Care Plan, the highly-rated health plan owned by Broward Health and Memorial Healthcare System, has partnered with CareSignal, a Deviceless Remote Patient Monitoring™ company, to increase Medicaid members’ access to care from the safety and convenience of home. Community Care Plan will use CareSignal’s platform to send accessible text-messages that will empower members to self-manage chronic conditions, communicate symptoms in real time, and connect to Community Care Plan’s Concierge Care Coordination team for additional support.

With members at home during COVID-19, the Deviceless Remote Monitoring program will enable timely identification of members who have immediate health needs. CareSignal’s automated Deviceless Remote Patient Monitoring program will also help CCP’s Concierge Care Coordination team scale to meet the increased need for remote care during the current pandemic and beyond.

“Community Care Plan strives every day to positively impact the health and wellness of those we serve,'' said Dr. Miguel Venereo, Chief Medical Officer, Community Care Plan. “Working with CareSignal to enhance member outreach and engagement through innovative communications will allow early identification and rapid intervention to prevent the deterioration of chronic conditions.  We anticipate that utilizing these enhancements will lead to reduced emergency department (ED) visits and admissions; therefore, improving the health outcomes and quality of life of our high-risk members.”

Community Care Plan members who enroll in the program will be able to use their existing phones to increase self-management for three chronic conditions and be just a text away from CCP’s Concierge Care Coordination team at any time. CareSignal will enroll members through convenient text-messages and a self-enrollment portal, as well as personal phone calls and mailers, which will be available in both English and Spanish. The program will focus on three conditions:
  • Asthma
  • Diabetes
  • Hypertension

Members will receive condition-specific questions and educational text messages or calls to their phones asking them to report symptoms and biometrics. To ensure equitable access, members can engage with the program using any phone, including landlines, and are not required to have a smartphone or WIFI to access the program. CCP’s Concierge Care Coordination team will provide support or route members to the appropriate site of service to improve their health.

Blake Marggraff, CEO of CareSignal, said “Community Care Plan’s actions show their commitment to their mission and proactively supporting its Medicaid members with chronic conditions during the pandemic. By combining automated remote monitoring with thoughtfully designed care coordination and support services, I am confident that CareSignal and Community Care Plan will simultaneously do good and do well. In partnering, our companies have aligned to achieve the combined goals of improving HEDIS measures, reducing avoidable hospitalizations, and connecting the right support with members who have pressing needs, right away.”
