Partnership connects Stratum Med’s over 12,000 physicians and 100 healthcare organizations with CareSignal’s clinically-validated technology for scalable remote monitoring to accelerate success in value-based care.

CareSignal® and Stratum Med, Inc. announce a strategic channel partnership to align the national network of innovative Stratum shareholders and members with Deviceless Remote Patient Monitoring® (Deviceless RPM®). This relationship connects Stratum’s provider organizations pioneering value-based care with the robust peer-reviewed evidence and real-world ROI of CareSignal Deviceless RPM technology for patient populations that have the most to gain.

Stratum and CareSignal already serve organizations including Esse Health (Missouri), Mankato Clinic (Minnesota), and Christie Clinic (Illinois) through this partnership, and the leadership of both organizations share the same vision of success. “Stratum is a role model for sustainable care delivery innovation,” said Blake Marggraff, CEO and co-founder of CareSignal. “The Stratum leadership team brings operational, clinical, and financial value into harmony, and their members’ stellar performance is evidence of the merit of their approach.”

Stratum views CareSignal as a timely partner with the right combination of capabilities to serve diverse organizations at a critical moment in healthcare. “CareSignal offers RPM with proven ROI for value-based populations and their providers,” said Lucas Thurman, Executive Director of Partnerships at Stratum. “This is more than an AI-powered engagement solution. This is a clinically proven technology paired with implementation services, shown to improve clinical outcomes for some of the most challenging disease states and patients.”

The COVID-19 pandemic placed a spotlight on the importance of healthcare at home, and also on the need for health equity, accessibility, and ongoing real-time data. Population health now demands rising-risk population support, including tech-enabled care management focused on chronic conditions, behavioral health conditions, maternal health, and social determinants of health. Moving forward, Stratum and CareSignal continue to collaborate with independent physician practices and provider organizations spanning a combined 21 states to bring the analytical, operational, and technological expertise required to succeed in the next era of healthcare.
